BRINGING VACCINES TO YOU.\nIf you would like to participate in the immunization clinic\, please complete the online format at: registration.vaccinateindiana.org/appointment/en/clinic/search Select: Jean Shepherd Community Center \nWalk-ins the day of the clinic are welcome\, but we MUST have parent/guardian consent if under the age of 18. Please upload or send the immunization records for the individual who is getting vaccinated. Please bring your insurance cards. We accept and bill all insurance (commercial\, Medicare\, Medicaid). There is no cost to those without insurance\, except for the Shingles and adult RSV vaccines\, as there is often a required co-pay. \nAll CDC recommended vaccines will be provided. We will consult the state immunization registry to assess the immunization needs of each individual.
